Job Overview


We are seeking a compassionate and organized Care Manager - Patient Care Coordinator to join our team at Apollo Health And Lifestyle Limited. As a Care Manager, you will be responsible for coordinating patient care, providing support to patients and their families, and ensuring the delivery of high-quality care. This is a full-time position based in Bangalore Rural, Karnataka, India.


Qualifications and Skills


  • Bachelor's degree
  • 1-3 years of experience in patient care coordination or a similar role
  • Excellent communication and interpersonal skills
  • Strong sales and customer service skills
  • Ability to work well in a team
  • Empathy and compassion for patients
  • Knowledge of counseling and patient education techniques
  • Attention to detail and strong organizational skills
  • Proficiency in medical terminology and electronic medical records


Roles and Responsibilities


  • Coordinate and manage patient care from admission to discharge
  • Assess patient needs and develop personalized care plans
  • Coordinate with healthcare professionals to ensure timely and appropriate care
  • Provide support and counseling to patients and their families
  • Educate patients on their health conditions and treatment options
  • Monitor patient progress and adjust care plans as necessary
  • Maintain accurate and up-to-date patient records
  • Collaborate with the interdisciplinary team to improve patient outcomes
  • Ensure compliance with regulatory guidelines and standards
  • Participate in quality improvement initiatives
